Current Patient Records

Need to access your patient records? Current patients can request their records or release them to a third party. 

Remember, only you or your personal representative have the right to access your protected health information. Only you, your Mental Health Representative or Healthcare Representative can allow a 3rd party to access your private health records.


Inpatient Psychiatric Hospitalization

Bowen Health Psychiatric Hospital provides crisis intervention and stabilization for patients suffering with mental illness, suicidal thoughts or behaviors, and detoxification for substance use and alcohol addiction. 

If you choose to admit yourself into Bowen Health Psychiatric Hospital voluntarily, you’ll need to complete our voluntary admission packet.

To request the release of your records from Bowen Health Psychiatric Hospital, please complete the release form. Remember, only you, your Mental Health Representative or Healthcare Representative can allow a 3rd party to access your private health records. 


NEW PATIENTS

Prepare for Your First Visit

We are grateful you chose Bowen Health as your healthcare partner. Please complete all relevant registration documents before your first appointment. 

On the day of your appointment, please arrive 30 minutes before your appointment time so we can ensure all your information is correct. Be sure to bring your photo ID and insurance card to your appointment. 

Need Help?

If you have a question, need assistance filling out the registration documents, or want to review insurance and payment details, we’re happy to help! You can ask a Patient Services Representative when you check-in at your appointment or give us a call, Monday – Friday from 8am – 5pm.


Appoint a Healthcare Representative

An appointed healthcare representative can make healthcare decisions on your behalf if you are incapable of doing so. You can appoint a representative for mental healthcare only or both mental and physical healthcare. Select the necessary form to complete your request.


Telehealth Consent

Select services at Bowen Health are offered via telehealth. If you choose to conduct your appointment via telehealth, you are required to first submit a Telehealth Consent Form. 


Patient’s Rights and Responsibilities

As a patient, you have certain rights and responsibilities. Some rights are guaranteed under federal law, and some are Bowen Health’s pledge to ensure you are treated with dignity, compassion, and respect.

ASPIN Patient’s Rights

If you or your child is registered as a participant in the Department of Mental Health and Addiction (DMHA) Supported Clients (DSC) Program, you are entitled to certain rights during your treatment. Learn about your rights and responsibilities within the program by clicking the link below.


Financial Assistance Consideration

Depending on your family’s income, you may be eligible for fee assistance discounts. To be considered, complete a Financial Assistance Assessment by clicking the link below.


Patient Concerns and Support

We believe each patient should be empowered with the skills to navigate and advocate for their healthcare. If you believe your care has not been provided fairly, we will provide a Patient Advocate to guide you through the grievance process by emailing PatientExperience@BowenHealth.org. You may also call the Consumer Service Line for the Indiana Division of Mental Health at (800) 901-1133.

Please do not use these phone numbers for emergencies. If you or someone you know is in crisis, walk in our door, call/text 988 or 800-342-5653, or go to the nearest Emergency Room.
